Blaine Allen Forman

Blaine Allen Forman (Peanuts), 75, of Morgantown, passed away peacefully at his home on Monday, July 18, 2022 with his daughters by his side. He was born July 1, 1947 to Harold Forman and Sara (Rodgers) Smith who both precede him in death.

Peanuts was known and loved by many. He adored his cat, Pretty, and his best friend, Lucy Morrison. He enjoyed his bartending days at Gene's Beer Garden, where he was a giver of perfectly poured draft and sarcastic one-liners. He was a lifelong NASCAR fan.

Blaine is survived by his brother, Mike Forman of Leesburg, FL, daughters Tracy (Randy) Pavalok of Grant Town, WV, and Kerry (Joshua) Kidder of Fairmont, WV, grandsons Dylan and Kaden Pavalok and Miles Kidder, a niece, Kristy (Terry) Kelley and nephew, David Forman, as well as a great-niece and two great-nephews. He was also preceded in death by his sister-in-law, Patty Forman.

The family would like to thank Dr. Darrell Saunders and the staff and nurses at the Mon General Cancer Center for their care during his chemotherapy treatments over the past 3 years and the team at Amedisys Hospice Care for their assistance during his final days.

Hastings Funeral Home is in charge of cremation services and a private interment will follow at Beverly Hills Memorial Gardens.

A celebration of life will be held at a later date.
